Maryland’s Community First Choice (CFC) Program is an amazing program that provides basic services to support and enhance your living more independently in the community. For many, it can mean all the difference as it can pay for a caregiver to come to your home and help you with your day-to-day activities. It can pay for delivered meals and other supports, depending upon your needs.
Supports Planning is the glue that brings all of the various services together! Like many State programs, how you enroll and qualify for services, then actually receive services can be a complicated process. A Supports Planner is there to help you and/or your loved ones navigate through the entire process, from applying for enrollment, to getting services and beyond. Your Supports Planner is there to make sure that the CFC (Community First Choice) program works for you and becomes your advocate and partner in achieving your goals for independence.
A key part of this is to meet with you and those currently supporting you to create a holistic person-centered plan. This planning process helps to ensure that the services you ultimately receive best match your goals and aligns with your specific needs.
The overall goal is to help you and/or your loved one to live the life they want, safely and independently within the community. As such, we continue to monitor how you are doing and how your services are working for you with monthly contacts and quarterly visits.
Your WJCIS Supports Planner is here to serve you!
The easiest way to start the process is to reach out to Maryland’s Department of Health at (410) 767-1739 and let them know that you are interested in applying for Maryland’s Community First Choice (CFC) program. Someone will take your information and give you further instructions on how to proceed.

As part of the initial eligibility process, you will receive a welcome packet from Maryland’s Department of Health (MDH) that includes information about the available Supports Planning Agencies. We highly recommend reviewing all the included information and picking the agency that is the best fit for you.
Our hope is that you will find that WJ Community Integration Services (WJCIS) is the best Supports Planning Agency for you. Your next step will be to call us at (410) 402-5382 during our normal business hours of 8:30 am to 5:00 pm, Monday through Friday, and let the person that you speak with know that you have already received your initial packet from the State, and that you would like to choose WJCIS as your supports planning agency.
If you are unable to speak to a live person, please simply leave a message with your information and a good callback number, and someone will reach out to you in order to have WJCIS selected as your agency.
